Job summary

Palmetto Advanced Therapy Services, a division of Palmetto Primary Care Physicians, seeks a compassionate Physical Therapist to join our outpatient team in North Charleston, SC.

You will conduct comprehensive evaluations, develop individualized plans, and collaborate with physicians and PTAs to optimize patient outcomes. A DPT and SC license are required; CPR/BLS preferred. On-site clinic with growth opportunities.

Essential Duties and Responsibilities
  • Conduct comprehensive patient evaluations, including review of medical history, physical assessments, and functional testing, to establish diagnoses within the scope of physical therapy practice.
  • Develop, implement, and modify individualized treatment plans based on evaluation findings, evidence-based practice, and patient goals.
  • Administer physical therapy treatments and modalities, including therapeutic exercise, manual therapy, gait training, neuromuscular re-education, electrical stimulation, ultrasound, and other interventions as clinically indicated.
  • Monitor and document patient progress, adjusting treatment plans as needed to achieve optimal outcomes.
  • Educate patients and caregivers on home exercise programs, injury prevention, and self-management strategies.
  • Communicate and collaborate with physicians, case managers, and other healthcare professionals regarding patient status, progress, and treatment recommendations.
  • Maintain accurate, timely, and complete clinical documentation in compliance with state regulations, payer requirements, and facility policy.
  • Supervise and delegate appropriate tasks to Physical Therapist Assistants (PTAs), in accordance with South Carolina Board of Physical Therapy Examiners supervision requirements.
  • Establish discharge plans and coordinate continuity of care, including referrals to other services as appropriate.
  • Ensure compliance with all applicable federal, state, and local regulations, including HIPAA, OSHA, and South Carolina physical therapy practice act requirements.
  • Participate in infection control, safety, and emergency preparedness protocols.
  • Maintain current licensure, CPR certification, and required continuing education credits.
Non-Essential Duties and Responsibilities
  • Participate in departmental meetings, in-service training, and quality improvement initiatives.
  • Assist with mentoring new staff, students, or interns.
  • Contribute to marketing or community outreach efforts (e.g., health fairs, screenings).
  • Assist with equipment maintenance requests and inventory management.
  • Perform other duties as assigned
Requirements
Required Knowledge, Skills, and Abilities
Knowledge
  • Strong working knowledge of human anatomy, physiology, kinesiology, and neuromuscular/musculoskeletal conditions.
  • Knowledge of evidence-based physical therapy evaluation and treatment techniques.
  • Familiarity with South Carolina physical therapy practice act, licensure regulations, and supervision requirements for PTAs.
  • Understanding of HIPAA, patient privacy, and healthcare documentation standards.
  • Knowledge of electronic medical records (EMR) systems and billing/coding basics (e.g., CPT codes) as applicable.

  • Excellent clinical assessment and problem-solving skills.
  • Strong manual therapy and hands-on treatment technique.
  • Effective verbal and written communication skills for patient education and interdisciplinary collaboration.
  • Time management skills to handle a caseload efficiently while maintaining quality of care.

Physical Requirements
  • Ability to stand and walk for extended periods (up to and exceeding 75% of the workday).
  • Ability to lift, transfer, and assist patients weighing up to 50 lbs. independently, and more with assistance or equipment.
  • Ability to bend, stoop, kneel, crouch, and reach frequently.
  • Sufficient manual dexterity and hand-eye coordination to perform manual therapy techniques and operate therapeutic equipment.
  • Ability to push or pull equipment such as wheelchairs, treatment tables, or exercise equipment.
  • Visual and auditory acuity sufficient to observe patient conditions, read documentation, and respond to alarms or verbal cues.
  • Ability to physically assist patients with transfers, ambulation, and fall prevention, including sudden physical exertion if a patient begins to fall.

Work is performed in an outpatient clinic. The role may require occasional travel between facilities.

Minimum Qualifications
  • Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
  • Current, active license to practice as a Physical Therapist in the State of South Carolina, issued by the South Carolina Board of Physical Therapy Examiners.
  • Current CPR/BLS certification.
  • Prior clinical experience preferred.

An exempt position responsible for evaluating, planning, and providing physical therapy treatment to patients to help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. The Physical Therapist works with patients across the lifespan, developing individualized treatment plans based on each patient's diagnosis, physician referral, and personal goals. This role requires a current license to practice physical therapy in the State of South Carolina and adherence to all state practice act requirements, facility policies, and professional standards of care.
