Job summary

Carolina Physical Therapy and Sports Medicine in North Charleston, SC is seeking a Physical Therapist to assess patients, develop and implement treatment plans, and provide direct care within a collaborative team. You will document care, supervise assistants, and coordinate with physicians and insurers to ensure quality outcomes.

Qualifications include graduation from a CAPTE-accredited PT program, SC licensure or eligibility, CPR, and experience in exercise rehab and manual therapy.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician referral and patient records to determine required treatment.
• Provide direct patient care per established protocols and guidelines.
• Document findings, progress and patient instructions for caregivers.
• Supervise assistants and rehabilitation technicians during care.
• Develop goals and a comprehensive plan of care.
• Implement treatment program and deliver essential therapies.
• Communicate with patients, families, physicians and insurers.
• Ensure timely documentation including daily notes and progress reports.

Job Description

• Assess patients to develop and initiate treatment plans based on evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine required treatment
• Provide direct patient care per established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and build a comprehensive program to attain those goals
• Implement the physical therapy treatment program and provide essential treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' comp reports, progress reports, initial evaluations, discharge summaries, and incident reports

Qualifications

• Graduate from a CAPTE-accredited Physical Therapy program
• Current South Carolina PT license or eligible
• CPR certification

Additional Information

This position offers a sign-on bonus. The salary range begins at $75,000 and increases based on relevant experience. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. This position is also eligible for discretionary incentive compensation, subject to change under the Company's plan. Medical, dental, vision, 401(k), paid time off, and other benefits are available per the terms of the Company's plans.
