Job summary

Jobtailor in South Carolina seeks a licensed Physical Therapist to evaluate residents and develop personalized treatment plans to improve mobility and independence.

You will supervise PT assistants, coordinate with physicians and nursing staff, document progress, and participate in quality initiatives while upholding infection control and safety standards.

Qualifications

  • Must hold or be eligible for a South Carolina Physical Therapist license.
  • Bachelor’s degree in Physical Therapy; graduates of Masters or Doctorate Program in Physical Therapy preferred.
  • Proficient in Net Health and Point Click Care.
  • Licensed as a Physical Therapist in the state; license in good standing.
  • CPR certification, if required by licensure.

  • Document timely, accurate, compliant medical records.
  • Maintain a safe treatment environment through infection control and equipment maintenance.

Job description

  • Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies
  • Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence
  • Assist the nursing department with training of Restorative Aides
  • Supervise Physical Therapy Assistants in direct resident care and resident-related activities, following the state practice act
  • Communicate with physicians, nursing staff, interdisciplinary team members, residents, and families regarding progress, goals, and discharge planning
  • Participate in Resident Care Conferences, Utilization Review meetings, and Rehabilitation meetings as needed
  • Provide in-service training programs for facility staff
  • Complete timely, accurate, and compliant documentation in medical records and in accordance with facility, governmental, and third-party payer requirements
  • Maintain a safe treatment environment through infection control, safety, and equipment maintenance protocols
  • Educate residents, families, and caregivers on treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies
  • Recommend and facilitate ordering durable medical equipment such as splints, walkers, canes, elevated toilet seats, and grab bars
  • Maintain confidentiality of residents' and company information
  • Report department equipment problems for maintenance
  • Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development
  • Supervise PT assistants, aides, and students
  • Relate positively, effectively, and appropriately with residents, families, staff, and professional colleagues
Requirements
  • Degree in Physical Therapy
  • Holds (or is eligible for) a South Carolina Physical Therapist license
  • Bachelor’s degree in Physical Therapy; graduates of Masters or Doctorate Program in Physical Therapy preferred
  • Proficient in computer skills such as inputting data into programs for Net Health and Point Click Care
  • Licensed as a Physical Therapist in the state
  • Must maintain a license in good standing at all time with the state board
  • Must meet continuing education requirements per state practices
  • CPR certification, if required by licensure
  • Ability to read technical procedures and comprehend policy and procedure manuals
  • Ability to effectively present information and respond to questions from managers, coworkers and families
  • Ability to apply fractions, percentages, ratios and proportions to practical situations
  • Ability to solve practical problems and interpret written, oral, diagram, or schedule instructions
  • Ability to stand and walk frequently and sit occasionally; occasionally climb, balance, stoop, kneel, crouch, or crawl
  • Ability to reach, push, pull, lift, and manipulate objects and equipment, with exertion that may occasionally exceed 100 pounds, frequently exceed 50 pounds, or constantly exceed 20 pounds
  • Effective communication, including talking and hearing
  • Ability to perceive and respond to tactile, auditory, and visual environmental information
  • Ability to assist, support, and transport residents using safe resident handling practices
