Job summary

US Physical Therapy is seeking a motivated Physical Therapist to join our outpatient team. The PT will assess patients, develop treatment plans, and provide direct care following approved protocols, while coordinating with physicians and insurance providers.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapy Program.
• Current PA licensure as a Physical Therapist (PT).
• CPR certification.
• Ability to work autonomously in a clinical setting.
• Proficiency with EMR systems and MS Office.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician referrals and patient records to determine needed PT treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Supervise and coordinate care provided by assistants and rehabilitation technicians.
• Develop a written plan of care and goals based on evaluation findings; create a comprehensive program.
• Implement treatment plans and deliver essential physical therapy interventions.
• Communicate effectively with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes, progress reports, initial evaluations, discharge summaries, and incident reports.

Xcelerate Physical Therapy is looking for a motivated individual to join our team as aPhysical Therapist.

What are some of the things you will do:

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, progress reports, initial evaluations, discharge summaries, and incident reports

• Graduate from a CAPTE accredited Physical TherapyProgram
