Job summary

Xcelerate Physical Therapy is seeking a motivated Physical Therapist to join our Delaware County, PA team. You will assess patients, develop treatment plans, and provide direct care under established protocols. You will document progress and communicate with caregivers and physicians to support coordinated care.

Qualifications

• Graduate from a CAPTE accredited Physical Therapy Program.
• New or recent graduates are encouraged to apply.
• Current state licensure as Physical Therapist (PA).
• CPR certification.
• Ability to work efficiently/effectively in an autonomous environment.
• Basic PC skills including EMR system, database software, and MS Office.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review the physician's referral and patient records to determine required therapy.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Responsible for patient care performed by assistants and rehabilitation technicians.
• Complete a written plan of care and develop goals based on evaluation findings.
• Implement therapy program and provide essential physical therapy treatments.
• Communicate effectively with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes, progress reports, evaluations, discharge summaries, and incident reports.

Job Description

Xcelerate Physical Therapy is looking for a motivated individual to join our team as aPhysical Therapist.

What are some of the things you will do:

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, progress reports, initial evaluations, discharge summaries, and incident reports

Qualifications

• Current state of PA licensure as Physical Therapist (PT)
• CPR certification
