Job summary

Xcelerate Physical Therapy is seeking a motivated Physical Therapist to join our outpatient clinic specializing in orthopedics and sports medicine.

You will assess patients, review referrals, provide direct patient care, document progress, and contribute to a comprehensive treatment plan. The position supports mentoring, onboarding, and opportunities for professional development in a family-friendly environment in Delaware County.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapy program.
• Current PA licensure as Physical Therapist (PT).
• CPR certification.
• Ability to work autonomously.
• Strong documentation and communication skills.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician referral and patient records to determine PT treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Oversee patient care performed by assistants and rehabilitation technicians.
• Complete a written plan of care and develop goals.
• Implement PT treatment program and provide essential physical therapy treatments.
• Communicate effectively with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes and discharge summaries.

Job Description

Xcelerate Physical Therapy is looking for a motivated individual to join our team as a Physical Therapist.

What are some of the things you will do:

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, progress reports, initial evaluations, discharge summaries, and incident reports

Qualifications

• Graduate from a CAPTE accredited Physical TherapyProgram
• New or recent graduates are encouraged to apply
• Current state of PA licensure as Physical Therapist (PT)
• CPR certification
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office
