Job Details

• Evaluate clients' physical functioning in their home environments
• Develop personalized treatment plans with defined goals and visit schedules
• Implement care plans under physician direction and in collaboration with agency staff
• Coordinate with clients, families, and caregivers to support rehabilitation efforts
• Observe, document, and report patient progress and any changes to physicians and team members
• Collaborate with other professionals to establish rehabilitation and maintenance activities
• Provide care across various age groups, with a focus on pediatric populations
  
  

Skills You Need

• Current New York State Physical Therapy license and registration
• Minimum one year of PT experience in hospital or long-term care settings
• Pediatric experience required; multi-population experience (adult, geriatric) a plus
• Ability to travel to patient homes using public or private transportation, including in challenging environments
