What You Will Do

• Practice independently in the community as part of an interdisciplinary care team
• Deliver personalized nursing and care management to patients in their homes or care facilities
• Constantly evaluate evolving patient needs and respond with care plan adjustments

Qualifications

• Current license to practice as a Registered Nurse in New York State
• Minimum of one year nursing experience in a medical/surgical environment
• Valid driver's license or NYS Non-Driver photo ID card may be required
• For Hospice Only: Wound Care Certification required or willingness to obtain within one year of hire; Certification in Hospice and Palliative Care Nursing required

Pay Range per Visit: If applying for the per diem per visit position, the range is $69.34 - $110.00 per visit.

Compensation: $112,209.00 - $138,409.00 annually
