Job summary

INNOVATE REHAB AND WELLNESS in Natchitoches, LA is seeking a Physical Therapist to provide comprehensive patient care in a skilled nursing setting.

You will evaluate movement, develop individualized treatment plans, and work closely with an interdisciplinary team to restore function and quality of life. Schedule: Monday – Friday with flexible options; a $10,000 sign-on bonus is offered for eligible applicants, and the pay range is $45.00–$70.00 based on experience.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem‑solving, and patient‑care skills.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence‑based treatment plans.
• Provide hands‑on therapy: manual techniques, therapeutic exercise, and mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with all regulatory and ethical standards.
