About the Role

As a Physical Therapist, you will guide patients through their rehabilitation journey, whether recovering from neurological conditions or physical trauma. You will work closely with physicians, nurses, case managers, and families to deliver coordinated, high‑quality care and support patients’ extraordinary progress.

Key Responsibilities
  • Lead the care journey – prioritize, coordinate, and manage all aspects of rehabilitation.
  • Collaborate with interdisciplinary teammates to align goals and improve outcomes.
  • Deliver plan‑driven care – ensure therapy aligns with each patient’s individualized medical plan.
  • Promote safety – maintain a safe, supportive environment for patients, families, and staff.
  • Educate and engage – communicate progress, care plans, and next steps to patients and families.
  • Ensure continuity – deliver a seamless experience from evaluation through discharge.
  • Support your team – assist others and contribute to a collaborative culture.
  • Guide and mentor – provide oversight to ancillary staff and support team development.
  • Adapt care in real time – re‑assess progress and adjust treatment plans based on patient needs and CMS guidelines.
  • Document with accuracy – record evaluations, interventions, and outcomes clearly and timely.
  • Drive quality improvement – use data to support outcomes and enhance program performance.
  • Assess with expertise – perform age‑appropriate evaluations and support accreditation standards (CARF and more).
  • Maintain accurate daily statistics and billing documentation.
  • Contribute to quality management initiatives and performance improvement.
  • Support compliance with accreditation standards and regulatory requirements.
  • Serve as a clinical resource to leadership and team members.
Required Qualifications
  • Degree in Physical Therapy (BS, Master’s, or Doctorate).
  • Current Louisiana Physical Therapist license.
  • Basic Life Support (BLS/CPR) – American Heart Association.
  • Non‑Violent Crisis Intervention certification (or obtain upon hire).
Preferred Qualifications
  • Experience in a rehabilitation or similar clinical setting.
  • Certifications in specialized areas (manual therapy, dry needling, vestibular rehab, women’s health).
  • Commitment to ongoing professional development and learning.
