Job summary

VNA Health Group is hiring a full-time Physical Therapist to deliver in-home and community-based therapy in Monmouth County, NJ. You will evaluate patients, develop and adjust care plans with physicians, and monitor progress while documenting changes.

Key Responsibilities:

• Evaluate and treat patients based on individual physical therapy needs

• Develop and adjust care plans in collaboration with physicians

• Monitor patient progress and document changes in condition

• Ensure high-quality care while following safety protocols and attendance standards

What You Need to Succeed:

• Valid NJ Physical Therapy License

• 2-3 years of clinical or community experience preferred

• Strong communication, interpersonal, and computer skills

• Valid NJ driver’s license and reliable transportation

Working Conditions/Physical Demand:

Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
