Job summary

VNA Health Group in Bergen County, NJ, is seeking a Per-Diem Physical Therapist to provide in-home and community-based physical therapy services. You will assess patients, develop and adjust care plans, and document progress to support rehabilitation goals.

The role requires a valid NJ Physical Therapy License and 2–3 years of experience, with strong communication and computer skills plus a valid NJ driver’s license. This position offers flexibility and a collaborative team environment.
