Crest Haven Nursing and Rehab in Middle Township, New Jersey is seeking a dedicated Physical Therapist PT to join our team. You will assess residents, develop individualized treatment plans, and administer PT interventions to improve mobility, balance, gait, and pain management.

Responsibilities include documenting evaluations and progress, educating residents and staff, and collaborating with OTs and SLPs to ensure comprehensive care. NJ licensure is required and a Master’s degree in PT.

We are seeking a dedicated and compassionate Physical Therapist PT to join our team at Crest Haven Nursing and Rehab, a unique provider of hotel-style healthcare in Middle Township, New Jersey. As a Physical Therapist, you will play a vital role in ensuring our residents receive exceptional care and support to achieve their optimal physical function.

Responsibilities:

• Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.

• Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain.

• Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.

• Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.

• Document resident evaluations, treatment plans, daily and progress notes, and HMO/Manage care progress updates in EMRs (Net Health/PCC).

• Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.
