Job summary

Angels Care Home Health is hiring a Physical Therapist to plan, implement, and evaluate patient care plans in a home-health setting. You will develop individualized treatment programs based on medical referrals and patient evaluations, and collaborate with a broader care team to optimize outcomes.

Qualified candidates hold an Oklahoma PT license, have at least two years of experience, and maintain CPR and Hepatitis profiles.

Qualifications

• Licensed PT in Oklahoma with current credentials.
• Minimum two years of physical therapy experience.
• CPR certification and Hepatitis profile current.
• Knowledge of durable medical equipment.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain well-being.
• Plans, organizes, and conducts physical therapy treatment programs based on referrals and evaluations.
• Collaborates with an interdisciplinary team.
