Job summary

Phoenix of Maple Heights is seeking a dedicated Physical Therapist (PT) to join our rehab team in Maple Heights, Ohio. This full-time role focuses on helping residents regain strength, mobility, and independence in a SNF/LTC setting.

You will evaluate residents, develop treatment plans, collaborate with nurses, physicians, and the inter-disciplinary team, and document progress using Net Health and PointClickCare (PCC).

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Ohio Physical Therapist license.
• Strong clinical, communication, and interpersonal skills.
• Team-oriented approach to resident care.
• SNF/LTC experience is a plus.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in a SNF/LTC setting.
• Help residents improve strength, balance, mobility, transfers, and functional independence.
• Monitor progress and modify treatment plans as needed.
• Collaborate with nursing, therapy, physicians, and the interdisciplinary care team.
• Participate in care planning and discharge planning.
• Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
• Provide compassionate, resident-centered care.
