Job summary

Phoenix of Maple Heights in Maple Heights, OH is seeking a PRN Physical Therapist Assistant to join our in-house rehabilitation team. You will work alongside our Physical Therapists to help residents improve mobility, strength, function, and independence.

Responsibilities include providing treatments per plan of care, documenting progress in Net Health, and collaborating with the supervising PT and rehab team to deliver resident-centered care.

Qualifications

• Current Ohio PTA license in good standing.
• Graduate of an accredited PTA program.
• SNF/LTC experience is a plus.
• Experience with Net Health documentation is a plus.
• Strong communication and documentation skills; team-oriented.
