Job summary

Forever Staffing Healthcare Agency is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in California. This full-time position focuses on evaluating residents, creating individualized plans, and delivering interventions to improve mobility and independence in a skilled nursing/long-term care setting.

You will work with patients, families, nursing and rehab staff to ensure high-quality care and accurate documentation in EMR systems.

Qualifications

  • Graduate of an approved/accredited Physical Therapy program.
  • Current, active Physical Therapist license in state of practice.
  • CPR certification as required.
  • Strong communication, documentation, organizational and customer-service skills.
  • Ability to work effectively with patients, families and interdisciplinary teams.

Responsibilities

  • Evaluate patients and develop individualized treatment plans.
  • Provide skilled PT services to maximize mobility and independence.
  • Monitor progress and adjust plans as needed.
  • Complete evaluations, daily notes, summaries, and monthly reports.

About the Role

Physical Therapist (PT) – Full-Time

Department: Rehabilitation

Employment Type: Full-Time

FLSA Status: Non-Exempt

Setting: Skilled Nursing / Long-Term Care

Position Overview

We are seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, and help residents improve mobility, strength, independence, and overall quality of life. This position works closely with patients, families, nursing staff, rehabilitation professionals, and other members of the interdisciplinary care team.

Responsibilities
  • Perform comprehensive physical therapy evaluations based on physician orders, payer authorization, and facility guidelines.
  • Develop and implement individualized treatment plans based on each patient's diagnosis, needs, and functional goals.
  • Provide skilled physical therapy services designed to maximize mobility, function, safety, and independence.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Complete evaluations, daily progress notes, summaries, monthly reports, and other required clinical documentation.
  • Enter patient information accurately into EMR and rehabilitation systems.
  • Identify and screen residents who may benefit from physical therapy services.
  • Educate patients, families, caregivers, staff, and other healthcare professionals regarding treatment plans and functional outcomes.
Qualifications
Physical Requirements

This position requires the ability to move throughout the workday and assist residents with mobility and therapeutic activities. Duties may involve extended periods of standing, walking, or sitting, as well as bending, squatting, pushing, pulling, and lifting or assisting with patients as appropriate.

Required Skills
  • Required:
  • Graduate of an approved/accredited Physical Therapy program.
  • Current, active, and unrestricted Physical Therapist license in the state of practice.
  • CPR certification as required.
  • Strong communication, documentation, organizational, and customer-service skills.
  • Ability to work effectively with patients, families, caregivers, and interdisciplinary healthcare teams.
Preferred Skills
  • Preferred:
  • Previous experience in long-term care or skilled nursing.
  • Knowledge of PDPM.
  • Knowledge of Medicare and other payer sources.
  • Experience with NetHealth and PointClickCare (PCC).
  • Knowledge of group and sensory therapy.
