Physical Therapist Position

We are seeking a skilled Physical Therapist to provide comprehensive PT services in a home health setting across California. The role involves assessing patients, developing individualized treatment plans, and collaborating with healthcare teams to promote functional recovery.

Compensation

• Hourly and Paid Per Visit (PPV) hybrid model, negotiable based on experience, skill level, and availability.
• Opportunity to accumulate overtime.

Essential Job Functions / Responsibilities

• Assess and evaluate therapeutic, rehabilitative, and functional status, including home environment.
• Assess muscle strength, mobility, gait, range of motion, and potential for rehabilitation.
• Direct and implement physical therapy treatments.
• Instruct patients and families/caregivers in the use and care of therapeutic appliances.
• Determine priority needs for physical therapy.
• Report patient reactions to treatment or changes in condition to the prescribing physician.
• Initiate physical therapy programs and instruct other personnel or family/caregivers as needed.
• Train patients in the use of prosthetic devices.
• Identify patient and caregiver needs for other home health services and make referrals as appropriate.
• Prepare and submit clinical and progress summaries based on goal attainment.
• Participate in patient discharge planning.
• Provide physical therapy consultation to families/caregivers when indicated.
• Provide in-service education programs for nursing personnel as needed.
• Participate in peer consultation.
• Supervise physical therapy assistants according to organizational policy and state regulations.
• Instruct, supervise, and evaluate home health aide care every two weeks when therapy is the only skilled service.

Position Qualifications

• Master’s or doctoral degree in Physical Therapy from an accredited institution.
• Licensed to practice as a Physical Therapist in California.
• Minimum of one year of experience as a Physical Therapist; community/home health experience preferred.
• Strong verbal and written communication and organizational skills.
• Current CPR certification.
• Licensed driver with a reliable, insured vehicle in accordance with organizational requirements.
