Overview

Job Title: Physical Therapist (PT) – Outpatient & Skilled Nursing

Location: Quincy & Greenville, CA (travel between sites as needed)

Employment Type: Full-time or Part-time (benefited)

Shift: Day shift

Position Overview

We are seeking dedicated and compassionate Physical Therapists to join our outpatient and skilled nursing care teams in Northern California. In this role, you will provide therapy services across multiple care settings, including outpatient, skilled nursing, inpatient, and swing bed departments. Both experienced PTs and those with at least 1 year of experience in acute, SNF, or outpatient care are encouraged to apply.

Key Responsibilities

• Evaluate, plan, and implement individualized physical therapy treatment plans
• Provide therapy services in outpatient, skilled nursing, inpatient, and swing bed settings
• Collaborate with interdisciplinary healthcare teams to achieve patient goals
• Document progress notes and maintain accurate medical records
• Educate patients and families on rehabilitation programs and safe mobility practices
• Maintain professional standards while delivering compassionate patient-centered care

Qualifications

• Bachelor’s degree (or higher) in Physical Therapy
• Active California Physical Therapy License
• Current BLS Certification (AHA approved)
• Passed the National Physical Therapist Examination (NPTE)
• Minimum of 1 year experience in acute, SNF, swingbed, or outpatient settings
• Competitive pay: $55.51 – $79.14/hour (based on experience)
• Full-time sign-on bonus: $10,000
• Part-time sign-on bonus: $5,000 (pro-rated)
• Housing support: 3 months paid housing OR stipend ($1,500/month full-time | $750/month part-time)
