Job summary

Quad C Physical Therapy Llc is seeking a Physical Therapist to join our team at two Los Angeles-area locations: Many, LA and Leesville, LA. We value a collaborative approach and high-quality care to help patients achieve their maximum function.

Responsibilities include comprehensive evaluations, goal setting with patients and care teams, ethical practice, timely documentation, and patient education through home exercise programs. New grads welcome alongside experienced therapists.

Qualifications

• Graduated from an accredited Physical Therapy Program.
• Current Louisiana Physical Therapy License.
• New Grads and Experienced Therapist applicable.

Responsibilities

• Provides patient care that is ETHICAL, in accordance to Physical Therapy Rules of Louisiana Board of Physical Therapy Examiners and APTA Standards of Practice.
• Performs a comprehensive evaluation/re-evaluation of patient in regards to physical therapy needs upon referral in a timely manner as determined by Quad C Physical Therapy's policy.
• Establish Short/Long Term Goals determined by evaluation in conjunction with patient, physician, caregiver, and additional disciplines involved with patient care.
• Provides interventions that require clinical skills and knowledge of a Physical Therapist consistent with established goals.
• Provides documentation in a timely manner for inclusion in patients medical record per Quad C Physical Therapy's policy.
• Maintains ongoing effective communication with patient, physical, caregiver, and additional disciplines and staff involved with patient care.
• Provides ongoing education and instruction through home exercise program to the patient and caregiver.
