Job summary

Peak Performance Physical Therapy in Louisiana is seeking a full-time Physical Therapist for our Brusly/Port Allen clinic. You will focus on patient experience in a team-driven environment and provide evidence-based outpatient care.

Responsibilities include evaluating patients, developing treatment plans, coordinating with physicians and insurers, documenting progress, and mentoring assistants. Competitive compensation ranges from $75,000 to $90,000.

Qualifications

• Graduate from CAPTE-accredited Physical Therapy program.
• Current LA license or ability to obtain; CPR certification.
• Able to create outpatient balance and vestibular rehab plans.
• Work efficiently in an autonomous environment.

Responsibilities

• Assesses patients and initiates treatment plans based on evaluation results.
• Reviews physician referrals, patient condition, and records to determine therapy.
• Provides direct patient care per protocols and guidelines.
• Documents findings, progress, and instructions to patients and caregivers.
• Ensure care delivered by assistants and rehab technicians.
• Develops plan of care, goals, and comprehensive program to attain goals.
• Implements therapy program and delivers essential treatments.
• Communicates with patients, caregivers, physicians, and insurers.
• Maintains timely documentation including notes and reports.

Job Description

We are seeking a highly motivated full-time Physical Therapist who is focused on patient experience and thrives in a team-first environment to join our exceptional clinicians at our Brusly / Port Allen clinic. Responsibilities

• Assesses patients and develops and initiates treatment plans based on the evaluation results.
• The Physician's referral (Prescription), the patient's condition, and the patient's medical records will be reviewed to determine the physical therapy treatment required.
• Provides direct patient care by established protocols and patient care guidelines.
• Documents findings, progress, and instructions to patients and caregivers.
• Responsible for patient care performed by assistants and rehabilitation technicians.
• Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals.
• Implements physical therapy treatment program and provides essential physical therapy treatments.
• Communicates effectively with patients/caregivers and professional colleagues, including physicians and insurance companies.
• Ensures timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.

Qualifications

• Graduate from CAPTE-accredited Physical Therapy program.
• A current state of Louisiana license or the ability to obtain, andCPR certification.
• Able to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans.
• Ability to work efficiently/effectively in an autonomous environment.
• Possesses basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) systems, Database software, and MS Office.
