Job summary

Peak Performance Physical Therapy is seeking a dedicated full-time Physical Therapist for our Brusly/Port Allen clinic. The role centers on patient-focused care within a collaborative team of clinicians.

You will assess, plan and deliver therapy, documenting progress and communicating with patients and caregivers. Required CAPTE-accredited PT program, Louisiana license or eligibility, CPR certification, and ability to plan balance/vestibular rehab.

Qualifications

• Graduate from CAPTE-accredited Physical Therapy program.
• Louisiana license or ability to obtain, and CPR certification.
• Able to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans.
• Autonomous work environment capability.
• Experience with EMR, databases and MS Office.

Responsibilities

• Assesses patients and develops and initiates treatment plans based on the evaluation results.
• Review physician prescription, patient condition and records to determine required therapy.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress and instructions to patients and caregivers.
• Supervise assistants and rehabilitation technicians in patient care.
• Complete written plan of care and set goals based on evaluation findings.
• Implement therapy program and provide essential treatments.
• Communicate with patients, caregivers, physicians and insurers.
• Ensure timely documentation including notes, progress and discharge summaries.

Job Description

We are seeking a highly motivated full-time Physical Therapist who is focused on patient experience and thrives in a team-first environment to join our exceptional clinicians at our Brusly / Port Allen clinic. Responsibilities

• Assesses patients and develops and initiates treatment plans based on the evaluation results.
• The Physician's referral (Prescription), the patient's condition, and the patient's medical records will be reviewed to determine the physical therapy treatment required.
• Provides direct patient care by established protocols and patient care guidelines.
• Documents findings, progress, and instructions to patients and caregivers.
• Responsible for patient care performed by assistants and rehabilitation technicians.
• Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals.
• Implements physical therapy treatment program and provides essential physical therapy treatments.
• Communicates effectively with patients/caregivers and professional colleagues, including physicians and insurance companies.
• Ensures timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.

Qualifications

• Graduate from CAPTE-accredited Physical Therapy program.
• A current state of Louisiana license or the ability to obtain, and CPR certification.
• Able to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans.
• Ability to work efficiently/effectively in an autonomous environment.
• Possesses basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) systems, Database software, and MS Office.
