Job summary

H2 Health in Live Oak, FL is seeking a Physical Therapist (PT) for our outpatient clinic. You will provide hands-on, evidence-based therapy, focusing on orthopedic, neurological, and post-operative care while building relationships with patients.

You will document care in Raintree EMR, manage about 50 visits per week, and work with AI-driven tools to streamline documentation, supported by a clinician-led team and opportunities for growth.

Qualifications

• Degree from a CAPTE-accredited Physical Therapy program.
• Active state Physical Therapy license or eligibility for licensure.
• Strong communication, interpersonal, and clinical reasoning skills.
