Job summary

Kadima Rehabilitation & Nursing at Latrobe is seeking a PRN Physical Therapist to join our in-house rehab team at the SNF/LTC facility. You will assess residents, plan treatment, and help restore function in a compassionate, patient-centered environment.

Responsibilities include evaluating and treating residents per a plan of care, documenting progress, and educating families and caregivers. PA licensure in good standing is required.

Qualifications

• SNF/Long-term care experience preferred.
• Current knowledge of PT treatment practices.
• Familiarity with Net Health Electronic Documentation System.

Seeking a PRN Physical Therapist to join in-house rehab team at Kadima Rehabilitation & Nursing at Latrobe SNF/LTC

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills:

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
