Job summary

Kadima Rehabilitation & Nursing at Latrobe is seeking a full-time Physical Therapist to join our in-house rehab team at our SNF/LTC facility. You will plan and administer treatments to promote recovery and improve function, aligned with individualized care plans.

The role requires current PA Physical Therapist license in good standing, experience in SNF/long-term care preferred, and strong clinical knowledge.

Qualifications

• SNF/Long-term care experience preferred.
• Current knowledge of PT treatment practices.
• Ability to manage patients with different personalities.
• Accurate and timely documentation of progress notes and plans of care.

Responsibilities

• Evaluate, plan, and administer treatment to residents and patients.
• Ensure treatment aligns with an established plan of care.
• Provide clinical support and instruct residents, families, and caregivers.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and modify treatments as needed.
• Complete documentation per company policy and state/federal regulations.

We are seeking a full-time Physical Therapist to join our amazing in-house rehab team at Kadima Rehabilitation & Nursing at Latrobe SNF/LTC

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills:

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
