Job Title: Home Health Physical Therapist

Location: Lady Lake, Florida

Reports To: Rehabilitation Director

Job Purpose

To deliver high-quality, evidence-based physical therapy services to patients in the home setting. This role includes direct clinical care, patient education, care coordination, and the supervision of Physical Therapy Assistants (PTAs) and other paraprofessionals.

Key Responsibilities

• Provide therapy services following agency standards and accepted clinical practices.
• Perform patient admissions, including OASIS assessments, and establish individualized care plans in collaboration with physicians, patients, and caregivers.
• Identify barriers to progress, develop measurable goals, and revise care plans as needed.
• Deliver therapeutic treatments and educate patients and caregivers regarding home exercises and recovery.
• Document care clearly and in a timely manner as per agency policy.
• Evaluate patient equipment needs and assist in implementation to improve functional outcomes.
• Communicate patient status and care plan updates to the interdisciplinary team and physician.
• Participate in discharge planning and prevent unnecessary hospitalizations or re-hospitalizations.
• Supervise Physical Therapy Assistants, Home Health Aides, and develop care plans for aide services.
• Ensure staff competency and conduct in-home supervision.
• Follow all infection control protocols and contribute to the agency’s Quality Assessment/Performance Improvement (QAPI) efforts.
• Attend in-service and continuing education programs.
• Perform additional duties as assigned.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist licensed in the state of Florida.
• Minimum of two years' experience in a healthcare setting.
• Ability to read and document in legible 12-point type.
• Effective verbal and written communication skills.
• Capable of physical activities including lifting, kneeling, climbing stairs, and extended travel between patient homes.
• Reliable transportation with valid driver’s license and insurance.

Work Environment & Physical Demands

• Must be able to work in varied home environments, which may include stairs, pets, and unique patient conditions.
• Must be comfortable working with patients who may be ill, disabled, or emotionally distressed.
