Job summary

Lacey Post Acute & Rehab in Lacey, WA is seeking a compassionate Physical Therapist to join our in-house therapy team at a 120-bed skilled nursing facility.

As a PT, you will conduct assessments, provide direct patient care, and document in PCC EMR; you will collaborate with an experienced team to promote functional independence and mobility for residents.

Qualifications

• Graduated from an accredited Physical Therapy program and licensed in WA.
• Proficient in PT assessments of referred patients with direct patient care.

Responsibilities

• Conduct Physical Therapy assessments of referred patients providing direct patient care.
• Provide PT services to residents to improve mobility and function.
• Document progress and outcomes in PCC EMR; collaborate with rehab team.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.

• Valid Physical Therapy (PT) license for state practicing.

• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.

• Proficient in EMR documentation (we use PCC but experience in this system is not required).

• CPR Certification.
