Job summary

Unifi Healthcare is seeking a Physical Therapist to evaluate patients, design individualized care plans, and deliver hands-on therapy in a home-health setting.

You will collaborate with case managers, respiratory therapy, and other staff to ensure coordinated, high-quality care and documentation. A CAPTE/APTA‑accredited degree, active license, and BLS are required.

Qualifications

• Graduate of a CAPTE/APTA-accredited PT program.
• Active PT license (or eligibility) in the state of hire.
• BLS certification (or willingness to obtain).
• A patient-first mindset and strong communication.

Responsibilities
