Eagle Ridge Health & Wellness – Keota, IA and Foxglove Wellness & Rehabilitation – Sigourney, IA are seeking a dedicated Physical Therapist (PT) to join our therapy team. You will help restore strength, mobility, and independence for residents through evidence-based plans of care.

You will work in a collaborative, interdisciplinary environment with a focus on dignity, safety, and quality care. Competitive compensation and strong teamwork are emphasized.

Qualificações

• Current Iowa Physical Therapist license.
• Strong clinical reasoning and assessment skills.

Responsabilidades

• Evaluate residents to determine functional abilities, rehabilitation needs, and individualized treatment goals.
• Develop and implement evidence-based physical therapy plans of care.
• Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions.
• Educate residents, families, and staff on safe mobility techniques and rehabilitation strategies.
• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely.
• Collaborate with nursing, occupational therapy, speech therapy, and administrative teams to ensure coordinated, high-quality care.
• Participate in care plan meetings and contribute to interdisciplinary discussions.
• Maintain compliance with all regulatory, clinical, and safety standards.
