Eagle Ridge Health & Wellness – Keota, Iowa and Foxglove Wellness & Rehabilitation – Sigourney, Iowa are seeking a dedicated Physical Therapist (PT) to join our therapy teams to restore residents' strength, mobility, and independence.

You will collaborate with nursing, OT, speech, and administrative staff to deliver high-quality, resident-centered care, participate in care plans, and uphold regulatory standards while enjoying competitive compensation and flexible scheduling.

Qualifications

• Iowa PT license is required.
• Strong clinical reasoning and assessment skills are essential.

Responsibilities

• Evaluate residents to determine functional abilities and rehab needs.
• Develop and implement evidence-based physical therapy plans of care.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
• Educate residents, families, and staff on safe mobility techniques.
• Document evaluations, treatments, progress notes, and discharge summaries.
• Collaborate with nursing, OT, speech therapy, and admin teams for coordinated care.
• Participate in care plan meetings and interdisciplinary discussions.
• Maintain compliance with regulatory, clinical, and safety standards.

Your Responsibilities

• Evaluate residents to determine functional abilities, rehabilitation needs, and individualized treatment goals
• Develop and implement evidence‑based physical therapy plans of care
• Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions
• Educate residents, families, and staff on safe mobility techniques and rehabilitation strategies
• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely
• Collaborate with nursing, occupational therapy, speech therapy, and administrative teams to ensure coordinated, high‑quality care
• Participate in care plan meetings and contribute to interdisciplinary discussions
• Maintain compliance with all regulatory, clinical, and safety standards
