Job summary

Eagle Ridge Health & Wellness and Foxglove Wellness & Rehabilitation are seeking an Occupational Therapist (OT) to join their therapy team in Keota and Sigourney, Iowa. The role focuses on helping residents regain independence and daily function through evidence-based OT plans of care.

You will collaborate with nursing, physical therapy, speech therapy, and administration to ensure high-quality, resident-centered care, with a strong emphasis on compassion, dignity, and teamwork.

Qualifications

• Iowa OT license or eligible for licensure.
• Experience in geriatrics or skilled nursing is preferred.
• Strong communication and teamwork skills.

Responsibilities

• Evaluate residents to determine functional abilities and rehab goals.
• Develop and implement OT plans of care.
• Provide interventions focused on ADLs, fine motor, cognition, and safe daily routines.
• Train residents in compensatory strategies to maximize independence.
• Educate residents, families, and staff on safe techniques and adaptive equipment.
• Document evaluations, treatments, progress notes, and discharge summaries.
• Collaborate with nursing, PT, speech therapy, and administrative teams for coordinated care.
• Participate in care plan meetings and interdisciplinary discussions.
• Maintain compliance with regulatory, clinical, and safety standards.

Your Responsibilities

• Provide therapeutic interventions focused on ADLs, fine motor skills, cognitive functioning, adaptive equipment use, and safe engagement in daily routines

• Train residents in compensatory strategies to maximize independence

• Educate residents, families, and staff on safe techniques, adaptive equipment, and functional support strategies

• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely

• Collaborate with nursing, physical therapy, speech therapy, and administrative teams to ensure coordinated, high-quality care

• Participate in care plan meetings and contribute to interdisciplinary discussions

• Maintain compliance with all regulatory, clinical, and safety standards
