Schedule: Monday through Friday

Plans and implements specific treatment programs for individual patients/residents according to the principles and practices of physical therapy.

Primary Duties

• 1. Evaluates assigned patients/residents and develops treatment plans, including specific goals and activities, reevaluating and modifying plans as necessary. Ensures treatments and care is done appropriately and completely in accordance with patients/residents clinical condition.
• 2. Provides instructions in therapeutic procedures to be continued by the patient as prescribed by a physician. Provides direct physical therapy to patients/residents.
• 3. Communicates clearly and tactfully with patient/resident regarding progress, problems, home programs and other issues related to the therapeutic process.
• 4. Communicates observations, facts and comments with interdisciplinary team members individually, and through team meetings as appropriate and required.
• 5. Assesses patient's progress weekly or as required and adjusts program as appropriate.
• 6. Prepares written documentation as requires by regulations and departmental policy, such as evaluations, individualized treatment plans, progress notes and other required reports
• 7. Provides direct Physical Therapy to patients per plan pf care.

• 9. Maintains established departmental policies, procedures, objectives, and quality assurance.
• 10. Performs above primary duties for ages adolescent through older adult.

Physical demands

• May be exposed to infectious and contagious diseases, body fluids and wastes
• Heavy physical effort, including lift/carry up to 50 lbs
• Prolonged standing/walking, bending and squatting
• Must be able to lift, position and transfer patients/residents and equipment

Minimum Education/Certification/Experience

• Current California Physical Therapy License
