William Penn Care Center in Jeannette, PA is seeking a dedicated Physical Therapist to join our team. You will develop and implement individualized therapy plans to improve residents' mobility, strength, and independence, working with an interdisciplinary team to deliver high-quality care.
The successful candidate will hold PA licensure, have a Bachelor's or Master's in Physical Therapy, and 1–2 years of long-term care experience is preferred.
We are seeking a dedicated and compassionate Physical Therapist to join our team at the William Penn Care Center in Jeannette, Pennsylvania. As a Physical Therapist at our center, you will have the opportunity to make a meaningful difference in the lives of our residents by providing expert care and support.
As a Physical Therapist, you will be responsible for developing and implementing individualized therapy plans for our residents, with a focus on improving their mobility, strength, and overall well-being. You will work closely with our interdisciplinary team to ensure that each resident's needs are met and that they receive the highest level of care possible. Your expertise will be invaluable in helping our residents recover from injuries, manage chronic conditions, and maintain their independence.
Assess and evaluate the physical abilities and needs of our residents
Develop and implement individualized therapy plans to achieve maximum functional independence
Conduct physical therapy treatments, including exercises, stretches, and other interventions
Monitor and record resident progress and adjust therapy plans as needed
Collaborate with other healthcare professionals, including doctors, nurses, and occupational therapists, to ensure comprehensive care
Maintain accurate and complete records of resident treatment and progress
Participate in interprofessional rounds and meetings to discuss resident care and goals
Current licensure as a Physical Therapist in the state of Pennsylvania
Bachelor's or Master's degree in Physical Therapy from an accredited program
Minimum of 1-2 years of experience in a long-term care setting, preferred
