Job summary

Hayward Gardens Post Acute in Hayward, CA is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. This full-time role offers $65.00-$75.00 per hour with comprehensive benefits and a collaborative environment.

The PT will evaluate residents, develop individualized treatment plans, and coordinate with families, physicians, and the interdisciplinary team to improve mobility and quality of life.

Qualifications

  • Current Physical Therapist license in the applicable state.

Responsibilities

  • Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and update individualized treatment plans focused on mobility, strength, balance, endurance, and functional independence.
  • Provide skilled physical therapy interventions and monitor resident progress toward established goals.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with applicable state practice requirements.
  • Assist the nursing department with training Restorative Aides.
  • Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding resident progress, goals, and discharge planning.
  • Participate in Resident Care Conferences, Utilization Review meetings, rehabilitation meetings, and other interdisciplinary meetings as needed.
  • Provide in-service training and education to facility staff as assigned.
  • Maintain timely, accurate, and compliant documentation in accordance with facility, governmental, and third‑party payer requirements.
  • Educate residents, families, and caregivers regarding treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention.
  • Recommend and facilitate appropriate durable medical equipment to promote safe mobility and independence, including walkers, canes, splints, elevated toilet seats, and grab bars.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance procedures.
  • Report equipment concerns promptly to ensure all department equipment remains safe and functional.
  • Maintain strict confidentiality of resident and company information.
  • Participate in quality assurance, performance improvement initiatives, and rehabilitation program development as assigned.
  • Build positive and professional relationships with residents, families, staff, and healthcare colleagues.

Hayward Gardens Post Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team.

The Staff Physical Therapist evaluates and treats residents, develops individualized treatment plans, collaborates with residents, families, physicians, and interdisciplinary team members, and helps residents achieve greater mobility, independence, and quality of life.

What You’ll Do
