Join Performance Optimal Health as an Outpatient Physical Therapist. Key Responsibilities & Collaboration

  • Be an integral part of the Client Experience, working closely with patients throughout their health journey.
  • Deliver empathetic, high-quality clinical care.
  • Establish personalized care plans to meet goals considering age, medical history, and condition.
  • Ensure clinical excellence through outcome assessments, mentorship, and ongoing education.
  • Collaborate with team members to provide integrated care and follow-up.
  • Apply professional competence and clinical reasoning in assessing patient conditions at all phases of therapy.
  • Partner with senior leadership to fulfill mandatory compliance requirements.
  • Supervise therapy aides appropriately, delegating patient and non-patient tasks.
  • Maintain a safe, clean work environment by adhering to procedures and regulations.
  • Accurately document patient care in our EMR system.
  • Protect patient confidentiality and organizational operations.
  • Communicate regularly with referring physicians regarding patient plans and updates.

Qualifications

  • 0-4 years of experience; valid State PT license (recent graduates welcome).
  • Graduated from an accredited DPT program.
  • Orthopedic experience, including post-operative cases, sports injuries, overuse syndromes, and back/neck conditions.
  • Board Certification in Orthopedics or Sports Physical Therapy is a plus.
  • Ability to work independently and efficiently, managing your own 'business unit.'
