We are seeking a Physical Therapist Case Manager for a remote position from California, USA. In this role, you will provide skilled therapy services to patients in their homes, focusing on mobility, rehabilitation, and independence. Your responsibilities include assessing patient conditions, developing personalized treatment plans, and collaborating with an interdisciplinary team to deliver high-quality care. Strong clinical judgment, excellent communication, and a compassionate approach are essential. You may also supervise Physical Therapy Assistants (PTAs) in accordance with standards. Your main goal is to help patients regain functionality and improve their quality of life at home.

Accountabilities

• Conduct home-based physical therapy evaluations and create individualized care plans.
• Implement treatment programs to restore mobility and prevent disability.
• Collaborate with physicians and healthcare professionals for comprehensive care.
• Supervise and guide PTAs to ensure adherence to treatment plans and best practices.
• Monitor patient progress, adjust care plans, and document accordingly.
• Educate patients and families on treatment, exercises, and lifestyle changes.

Requirements

• Active and unrestricted California Physical Therapist License.
• Current BLS/CPR certification.
• Valid California Driver's License.
• Knowledge of home health regulations and local community resources.
• Preferred: prior experience in home healthcare.
• Experience with high-acuity patients and interdisciplinary teams is a plus.
• Empathy, leadership, organization, and critical thinking skills.
