Park Place Transitional Care & Rehab in Great Falls, MT is hiring a Physical Therapist for our in-house therapy team. We treat ortho, cardiac, neuro and wound care patients, plus our ventilator unit, offering high-acuity respiratory skill-building on site.

You will screen and evaluate patients, provide skilled therapy, document progress, counsel families, communicate with the team, and complete discharge summaries.

Competenze

• Licensed Physical Therapist; Montana license or eligibility.
• All experience levels welcome; new grads welcome.

Mansioni

• Screen and evaluate patients with physical functioning disorders and/or wound care needs and develop a plan of care that meets regulatory and clinical standards.
• Provide skilled, medically necessary treatment, including manual techniques, ambulation, therapeutic exercise, modalities, assistive devices and wound care.
• Assess progress, adjust treatment and document in the clinical record.
• Counsel patients, families and caregivers.
• Communicate progress, barriers and plans with your supervisor and the team.
• Complete a full discharge summary.
