Job summary

Interim HealthCare Home Health and Hospice in Grand Haven, MI is seeking a Home Health Physical Therapist to provide personalized, home-based therapy to patients recovering mobility after illness or injury. You will work within a home health team to execute the plan of care and improve patient outcomes.

The role requires a graduate of an accredited Physical Therapist program with an active Michigan PT license and CPR certification, plus at least one year of relevant experience.

Qualifications

• Must be a graduate of an accredited Physical Therapist program and hold an active Michigan PT license.
• At least 1 year of clinical experience, preferably in home health, within the last 5 years.
• CPR certification is required.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.

Responsibilities

• Provide home-based physical therapy to patients with reduced mobility after illness or injury.
• Work as part of a home health team focused on the patient’s plan of care and goals.
• Observe movements, conduct assessments, document progress and modify the care plan as needed.
• Assist patients with exercises and hands-on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.

As a Home Health Physical Therapist, here's a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in Michigan(NEW GRADS WELCOME!)
• 1 year of clinical experience, preferably within home health in the last 5 years
• CPR certification
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
