Job summary

Enhance Rehabilitation in Glenwood, IL is seeking qualified Physical Therapists to join our team in a SNF setting. You will assess and treat patients following an individualized plan of care and contribute to high-quality outcomes.

You will evaluate new patients, develop plans of care, set measurable goals, educate patients and families, supervise Physical Therapist Assistants, and document progress and discharges accurately to support continuous improvement.

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
