Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a skilled nursing facility setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure the highest quality of care.

You will also supervise Physical Therapist Assistants and perform re-evaluations as needed. In this role you will educate patients and families about injury, progress and likely outcomes, while maintaining complete and accurate

Qualifications

• Licensed physical therapist in good standing.
• Experience in SNF or long-term care preferred.
• Ability to develop and monitor plans of care.

Responsibilities

• Provide excellent care for patients.
• Collaborate with facility staff to ensure quality care.
• Evaluate new patients and set measurable goals.
• Treat patients and educate them on injury, progress and outcomes.
• Supervise Physical Therapist Assistants.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
