Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following individualized plans of care, while coordinating with facility staff to maintain high standards of patient care.

Responsibilities include conducting patient evaluations, developing plans of care with functional goals, supervising PTAs, and ensuring timely, accurate documentation.

Qualifications

• Licensed Physical Therapist in good standing (state licensure required).
• Ability to evaluate patients and develop measurable plans of care.
• Strong documentation and communication skills.

Responsibilities

• Provide high-quality care to patients in SNF settings.
• Collaborate with facility staff to ensure excellent care.
• Evaluate new patients, develop plans of care and measurable goals.
• Treat patients and educate them on injury, progress and outcomes.
• Supervise Physical Therapist Assistants.
• Perform re-evaluations and update plans of care as needed.
• Complete timely, accurate documentation including evaluations and progress notes.

In this role, you will evaluate and treat patients following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
