Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following individualized plans of care.

As part of a growing team, you will collaborate with facility staff, educate patients and families, supervise Physical Therapist Assistants, and ensure accurate documentation of evaluations, progress notes, and discharges.

In this role, you will evaluate and treat patients following an individualized plan of care.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
