Fairview Hospital / Cleveland Clinic seeks a Home Health Physical Therapist to assess patients in their homes, design personalized plans to improve mobility, endurance, and pain, and deliver hands-on interventions. You will collaborate with physicians and the care team to optimize therapy, document progress, and educate families on care plans.

You will also ensure plans comply with state guidelines, complete evaluations, and maintain high-quality, equitable care in diverse home environments.

Qualifications

• Current state licensure in physical therapy.
• CPR certification.
