Job summary

Black Women's Mental Health Institute is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes in West Palm Beach, FL. You will evaluate mobility, develop personalized treatment plans, and support safe transitions of care.

You will work autonomously within a care team, educate patients and caregivers, document progress, and coordinate with nurses and physicians to optimize recovery and independence.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in practice state.
• 1–2 years of experience, preferably in home health.
• Strong clinical assessment, critical thinking, and communication skills.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities
