Job summary

AccordCare in Florida seeks a licensed Physical Therapist to deliver high-quality, compassionate care in patients’ homes throughout the state. You will perform assessments, develop individualized plans of care, and collaborate with an interdisciplinary team to ensure safe, effective therapy.

The role requires a Florida license, knowledge of OASIS/home health regulations, and strong communication skills. Travel within the service area is expected to support patient goals and continuity of care.

Qualifications

  • Current, active Physical Therapist license issued by the Florida Board.
  • Strong knowledge of OASIS and home health regulations.
  • One year of physical therapy experience (home health desirable).
  • Strong communication and documentation skills.

Responsibilities

  • Develop and implement individualized, goal oriented physical therapy plans of care.
  • Conduct initial and ongoing comprehensive patient assessments and evaluations, including OASIS C evaluations, coordinating with physicians before initiating care.
  • Determine scope and frequency of services based on acuity and patient/family needs with Clinical Manager.
  • Provide skilled therapeutic treatments, including pain management and restoration of function through heat therapy, ultrasound, and exercise programs.
  • Continuously assess patient responses and modify the Plan of Care; inform physician and staff of changes.
  • Develop comprehensive discharge plans based on interdisciplinary input.
  • Participate in in-service training and QAPI activities as requested.
  • Join interdisciplinary team meetings to coordinate care and seek support.

This role supports the delivery of safe, goal-oriented patient care, promotes positive relationships with patients and families, and contributes to the overall growth and success of the organization. This position functions within a Medicare-certified and AHCA-licensed home health agency serving patients in their homes or residential settings throughout the state of Florida.

Responsibilities
  • Develop and implement individualized, goal oriented physical therapy plans of care.
  • Conduct initial and ongoing comprehensive patient assessments and evaluations, which may include OASIS C evaluations, ensuring communication with physicians prior to initiating care.
  • Determines scope and frequency of services needed based on acuity and patient/family needs in consult with Clinical Manager.
  • Provide skilled therapeutic treatments, including pain management and restoration of function through various techniques such as heat therapy, ultrasound, and exercise programs, using proper body mechanics.
  • Continuously assess patient responses to therapy and recommend modifications to the Plan of Care as needed, report changes to the supervising Clinical Manager. Informs the physician and other personnel of changes in the patient’s needs.
  • Develop comprehensive discharge plans based on interdisciplinary input.
  • Participates in in-service training and quality assessment performance improvement (QAPI) as requested by the office.
  • Participate in interdisciplinary team meetings to coordinate care, problem-solve, and receive support and education.
Qualifications
  • Current, active Physical Therapist license issued by the Florida Board of Physical Therapy Practice and in good standing.
  • Strong knowledge of OASIS and home health regulations
  • One year of physical therapy experience (home health experience desirable).
  • Strong communication and documentation skills.
  • Ability to work collaboratively as part of a healthcare team.
  • Commitment to patient-centered, safe, and compassionate care.
Work Environment Considerations
Role
  • Safely lift, carry, push, or pull up to 50 pounds.
  • Assist with patient transfers and ambulation.
  • Tolerate prolonged periods of physical activity, including standing and walking.
  • Respond quickly in emergency situations.
  • Travel throughout the service area on a daily basis to visit patients/clients
OSHA Category 1

Tasks involve routine exposure to blood, body fluids, communicable diseases or other potentially infectious materials. Must follow universal precautions, infection control procedures and personal protective equipment (PPE) are required.
