Job summary

Proforma Health is seeking a Home Health Physical Therapist (PT) to provide personalized, home-based therapy focused on mobility, strength, and pain management. You will conduct assessments, develop and modify care plans, assist with exercises, and educate patients and families on goals and self-care.
