Responsibilities

Role:

Home Health Physical Therapist (PT)

Responsibilities:

Provide personalized home-based physical therapy to patients recovering mobility, strength, and function after illness or injury. Collaborate with a multidisciplinary team, conduct assessments, document progress, and adjust care plans accordingly. Assist patients with exercises and hands-on therapy to manage pain and improve mobility, while educating patients and their families on care plans and self-care practices.

Qualifications:

Graduate of an accredited Physical Therapist Program with an active PT license. Ideal candidates have at least 1 year of clinical experience, preferably in home health, and CPR certification. Must possess good clinical judgment, strong interpersonal skills, dexterity, and compassion. New grads are welcome.
