Job summary

Trinity Healthcare Talent Services is seeking a qualified Physical Therapist to join our SNF community in Virginia. The role involves evaluating patients, developing individualized treatment plans, and delivering hands-on rehabilitation to a diverse population.

You will work full-time, Monday through Friday with weekend rotation, holding a current Virginia physical therapy license and a degree in physical therapy from an accredited program. Prior SNF experience is preferred.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• Minimum of 1-2 years of experience in a similar setting preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct initial evaluations and assessments to determine patients' needs and develop individualized treatment plans.
• Provide hands-on therapy and exercises to improve patients' strength, mobility, and range of motion.
• Educate patients and their families on exercises and techniques to continue their rehabilitation at home.
• Monitor and document patients' progress and adjust treatment plans as needed.
• Collaborate with other healthcare professionals, including doctors and nurses, to ensure comprehensive care for patients.
• Communicate effectively with patients, families, and caregivers to provide support and guidance throughout the rehabilitation process.
• Adhere to all legal and ethical standards in patient care and documentation.
• Participate in continuing education and professional development opportunities to stay current with best practices and advancements in the field of physical therapy.
