Job summary

Trinity Healthcare Talent Services, LLC is seeking a Physical Therapist for a 100-bed skilled nursing facility. Compensation ranges from $46 to $51 per hour, with a 35-hour workweek from Monday to Friday and no weekend rotation.

You will conduct initial evaluations, develop individualized treatment plans, administer therapies, monitor progress, and collaborate with physicians and nurses to ensure comprehensive care. Education includes PT degree and state licensure; SNF experience preferred.

Qualifications

• PT degree from an accredited program (Bachelor's, Master's, or DPT).
• Current state Physical Therapist license.
• 1-2 years SNF experience preferred.

Responsibilities

Position: Physical Therapist

We are seeking a highly skilled and dedicated Physical Therapist to join our team at our 100-bed skilled nursing facility. As a Physical Therapist, you will work with a diverse patient population to provide rehabilitative services and improve their overall physical function and mobility.

Key Responsibilities:

• Conduct initial evaluations and develop individualized treatment plans for patients based on their specific needs and goals
• Administer therapeutic exercises, manual therapy techniques, and modalities to improve strength, range of motion, and functional abilities
• Monitor and document patient progress and adjust treatment plans as needed
• Collaborate with other healthcare professionals, such as doctors and nurses, to ensure comprehensive and coordinated care for patients
• Educate patients and their families on home exercise programs and strategies for maintaining their physical function and mobility
• Adhere to all state and federal regulations and ethical standards in the provision of physical therapy services

Qualifications:

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Current state license to practice as a Physical Therapist
• Minimum of 1-2 years of experience in a SNF setting preferred
• Strong knowledge of anatomy, kinesiology, and therapeutic techniques
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Basic computer skills and proficiency in electronic medical record systems
