Job summary

Total Care Therapy LLC in Fort Wayne, IN is seeking a compassionate Physical Therapist for a part-time role. You will evaluate residents, develop personalized plans, and provide skilled PT services to support mobility, function, and independence in assisted living and related settings.

Responsibilities include supervision of PTAs, documentation in EMR, and education of residents and caregivers. A valid PT license, CPR/BLS, and a valid driver's license are required, with opportunities to impact

• CPR/BLS Certification is required.
• Valid driver's license and reliable transportation are required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including exercise, gait, balance, transfer, and functional mobility interventions.
• Reassess progress and modify treatment plans as appropriate.
• Supervise PTAs per state regulations.
• Document using EMR in a timely and accurate manner.
• Educate residents and staff on treatment plans and home exercise programs.
• Collaborate with interdisciplinary team for coordinated care.

About the Role We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient‑centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
